Track Change Requests

Families wishing to request a different track assignment for the 2026-27 school year must complete and submit this online form. If you prefer a paper form, you may download one here. All forms (online or paper) must be received by 4:00 pm on Monday, February 2, 2026. Any forms (online or paper) received after that time will not be considered.

 

If you are requesting a track change for multiple siblings who will attend Salem Middle School, you must complete a separate form for each child. If you are also requesting a track change for a student at a year-round elementary school, you must ALSO submit a form to the elementary school. If you do not submit forms at both schools, we cannot guarantee that both students will be moved to the same track. Contact the elementary school for details on their track change process.
