Address Change for AFHS

• To complete an address change, families must provide the following:
  • School Assignment Form (Form 2805 - Routine) - click the link above
  • Proof of Residence - one of the following:
    • Electric, gas or water bill (dated within the last 30 days)
    • Lease agreement (signed within the last 60 days)
    • Settlement or closing statement (signed within 30 days in the name of the parent or court-appointed guardian)
  • Copy of the parent/guardian ID
  • Transportation Service Request Form (if needed)
• Families can submit the paperwork in-person or email ALL the documentation, including a copy of parent's ID, to JJWall@wcpss.net. If you have any questions, contact Jennapher Wall at 919-694-0500 ext. 20016.

• Wake Tech CCP- Dual Enrollment & Early Release/Late Arrival- ALL students participating in CCP must submit this two page form.
