Health Requirements
Parents or guardians are encouraged to provide proof of health assessment and/or immunizations to the school on or before the child's first day of attendance.
North Carolina Health Assessment
State law requires that every child entering public schools in North Carolina for the first time receive a health assessment. The assessment must occur within 12 months prior to entering school. Parents or guardians are encouraged to provide a North Carolina Health Assessment Transmittal Form completed by a medical provider to the school on or before the child's first day of attendance.
In accordance with state law, at the end of the 30th calendar day from the first day of attendance, any child without proof of a NC health assessment and/or immunizations shall be prohibited from attending school until the required proof is provided. Appointment cards can not be accepted as proof of immunization.
Immunizations
North Carolina General Statutes (G.S. 130A-155) requires immunizations for every child attending public school in this state. Parent(s), guardian(s) or person in loco parentis are responsible for ensuring that their child(ren) receives required North Carolina immunizations and must present proof of immunizations at the time of enrollment for the minimum requirements as specified in the law.
