Complete the 506 form to apply for the Wake County Indian Education Program. When you complete your form, here are your options for turning it in:
- Scan your form and email it to Becky Locklear at ralocklear@wcpss.net
- Call Becky Locklear at 919-694-0352 and set up a time to turn in your form in person.
- Mail your form to 5625 Dillard Dr., Cary, NC, with attention to Laura Abernethy, Family Engagement Coordinator.
- Rather than returning the form to your child's school, please reach out to Becky Locklear for assistance.
- This form only needs to be completed once and is valid as long as the student is registered in WCPSS.
