• Immunization and Health Requirements

    Parent/guardian(s) are encouraged to provide proof of health assessment and/or immunizations to the school on or before the child's first day of attendance.

    Following state law, at the end of the 30th calendar day from the first day of attendance, any child without proof of an NC health assessment and/or immunizations shall be prohibited from attending school until the required proof is provided. Appointment cards can not be accepted as proof of immunization.

    Health Assessments
    North Carolina State Law requires that every child entering a public school in North Carolina for the first time receive a health assessment. The assessment must occur within 12 months prior to entering school and be completed by a medical provider using a North Carolina Health Assessment Transmittal Form.

    Immunizations
    North Carolina General Statutes (G.S. 130A-155) requires immunizations for every child attending public school in North Carolina. Parent/guardian(s) or person in loco parentis are responsible for ensuring that their child(ren) receives required North Carolina immunizations and must present proof of immunizations at the time of enrollment for the minimum requirements as specified in the law.     Immunizations "In Process"
    Students who have begun a series of immunizations, but have not completed the series due to the need to meet minimum intervals between doses, are considered “in process” and may attend school. An appointment card containing the next immunization appointment date must be presented to the school as proof of “In Process” immunization. Any student without proof of their next scheduled immunization appointment or who does not attend their next scheduled immunization appointment shall be prohibited from attending school until the immunization has been obtained.

    Health Screenings

    Parents/guardian(s) may contact their student’s principal to opt out of hearing, vision, and/or dental screenings.

    The following screenings are provided to students at certain grade levels or schools. Students at any grade level with evidence of problems related to hearing, vision, dental health, communicable diseases, or related academic issues will be screened or re-screened. 

    • Hearing - Students at any grade level with evidence of problems related to hearing will be screened upon request. School audiologists oversee and provide follow-up for all failed hearing screenings.
    • Vision - Vision is screened by trained school staff for students in grades 1, 3, 5 and kindergarten if a vision screening is not documented on the North Carolina Health Assessment Transmittal Form. Screening is provided for students in other grade levels as referred by school staff. The school nurse is contacted if screening/re-screening or follow-up assistance is needed.
    • Dental - All kindergarten students receive a dental screening by a dental hygienist.

     
    Medication

    • Medication must be medically necessary during the school day 
    • Form 1702 is required for medication to be administered by school staff OR self-carried/self-administered by the student during the school day  
    • School nurses train school staff to administer medication 
    • Medication cannot be administered without a completed Form 1702 
    • If medication is not available, 911 will be called for emergencies 
    • Parent/guardian(s) are responsible for monitoring medication expiration dates and replacing medication as needed
    • Student Medication Administration Requirements
